To submit a DCFS Psychotropic Medication request, one must submit the CFS 431-A Psychotropic Medication Request available on the DCFS website. Go to the “Guardian Consent Portal” forms by selecting “ Submit a Request for Consent” and then select “CFS 431-A: Psychotropic Medication” to submit a online request or follow this link https://guardianconsent.dcfs.illinois.gov/RequestsForConsent/Psychotropic-Medication-Request/.

 

If you would like more guidance about online completion of the CFS 431-A, please review the Online CFS – 431-a Submission Instructions or watch the Online 431-A demonstration video. Use these resources especially if completing a one-time Emergency Medication notification because those need to be completed exactly as instructed which may be confusing.

 

If you need to request a hardcopy consent CFS 431-B, please call the DCFS Consent Unit at 800-828-2179. Alternatively, you may fax in a CFS 431-D Request for Copy of Psychotropic Medication (or Other*) Consent to 312-814-7015.

 

Rule 325

Per Illinois Rule 325, “DCFS specifically prohibits the use of pro re nata (PRN) medications. Prior consent for the one-time administration of a psychotropic medication is not necessary when an emergency exists. However, all administration of emergency medications must be reported to DCFS.”
