DCFS and Youthcare Information

To submit a DCFS Psychotropic Medication request, one must submit the CFS 431-A Psychotropic Medication Request . Alternatively, if you are unable to fax a request, you may email it to This email address is being protected from spambots. You need JavaScript enabled to view it..

If you need to request a hard copy consent, please call the DCFS Consent Unit at 800-828-2179. Alternatively, you may fax in a CFS 431-D Request for Copy of Psychotropic Medication (or Other*) Consent to 312-814-7015.

Rule 325
Per
Illinois Rule 325 , “DCFS specifically prohibits the use of pro re nata (PRNT) medications. Prior consent for the one-time administration of a psychotropic medication is not necessary when an emergency exists.  However, all administration of emergency medications must be reported to DCFS.”

Youthcare

In April 2020, youth in care switched from Medicaid Fee for Service to Youthcare, a manage care plan. The vast majority of youth in care are on the Youthcare plan with a few exceptions for medically complex youth. Please consult the Youthcare website for more in depth information about their services.
